Rejecting a campaign and suspending an ad (option B) or rejecting an ad and suspending a campaign (option D) are too limited; the issue often lies with the product itself, not just the ad or campaign. Rejecting a feed and suspending an item (option C) is also insufficient; while a feed might be rejected, the ultimate consequence for severe violations is an account block.
